As the Team Leader – Individualised Supports, you will be committed to ensuring that peoples’ individualised support requirements are fulfilled, with a strong focus on seeing people move towards personal goals.As a Team Leader, you will be responsible for coordinating and managing services requested by participants with the aim to create a seamless experience and deliver a high standard support.The successful candidate will report to Manager Disability Services and will lead and coach a team of Support Workers with a person-centred and customer-focused approach. You will have the initiative to work and solve problems unsupervised, with a strong focus on reaching set goals and targets.Essential Criteria:Demonstrated experience in service coordination or practical experience within the disability/human services sectorExperience in Disability Accommodation Management, SDS and Supported Independent LivingUnderstanding of the NDISValid NSW Driver Licence (minimum P2)Own vehicle with comprehensive car insuranceSatisfactory background check, including NDIS Worker Check and/or Working with Children CheckRight to work in AustraliaCurrent First Aid Certificate (HLTAID003)People of Aboriginal and Torres Strait Islander descent are highly encouraged to apply.Application close date: Friday, 15th October 2021.
